Medical Records: Records sent by email or directly to another healthcare provider are complimentary.
Speed Up Your Request: Include your email address on the authorization form to help expedite processing and delivery.
Disability/FMLA Forms: The first form is $45, with each additional form submitted at the same time costing $15.
Processing Time: Disability/FMLA forms are completed within three business days after payment is received.
