For local green card applicants, ExperCARE is your local solution to completing the I-693 form required by the U.S. Department of Homeland Security.
Our certified civil surgeons, Dr. J. David Carson and Dr. Robert Mazur, and experienced team ensure all medical requirements are completed to the satisfaction of USCIS. As an applicant, you can register at any of our clinic locations for an appointment seven days a week. After registration, we require 48-72 hours for our team to review your submission with the requirements of the I-693. ExperCARE offers labs and vaccines at clinic locations, making the process even more convenient! We also receive our lab results directly from Quest Diagnostics, keeping your process moving as quickly as possible.
Once all results are completed and/or turned to ExperCARE, you will be contacted for an appointment with Dr. Carson (Mall Boulevard clinic) or Dr. Mazur (Richmond Hill clinic) within two weeks. You also receive the completed I-693 form on the day of your appointment.

Services and Fees
Complete Immigration Physical Examination
The cost of the complete Immigration Physical Examination is $350.00 and includes:
- Physical Examination
- Allow up to 3 hours for the entire process
- Review of your written vaccination and other health records
- Syphilis test (RPR) – Required for most applicants 15 and older, or possibly younger based on the physician’s determination
- Certified and Sealed I-693 form with a copy for yourself
- Copies of your laboratory testing
- Due to the nature of the tests that may be required, you may need to make at least two visits or more to the clinic before completion of all facets of the exam
Additional Services That May Be Required
Additional services that may be required depending on your age but are not included in initial the exam cost include:
- TB (tuberculosis) screening test (IGRA blood test)
- Chest X Ray if the TB screening test is positive
- Urine gonorrhea testing
- Blood lab titers, if necessary
- It is up to the Civil Surgeon to determine if blood lab titers are required for your examination
- If vaccinations are required, most may be obtained in the clinic for an additional fee. If the vaccines are not available in the clinic, they may be obtained at your primary care physician or we can arrange for them at a local immunization clinic at an extra fee that will be paid to the clinic. We do have many, but not all, of the required immunizations at Pocono Urgent Care. Please call us for any questions.
What To Bring For An Immigration Physical
You will need to bring the following to your immigration physical:
- Valid, government issued picture ID such as a valid passport (preferred), driver’s license, green card, or military ID.
- Medical records (translated to English, if necessary), including proof of prior treatment for tuberculosis or other infectious disease and prior chest x-rays
- Immunization records (translated to English, if necessary)
- Any mental health records (translated to English, if necessary)
- An immigration physical form I-693, pre-completed with your information. Do not date it.
What To Expect After The Exam And Follow Up
The entire exam process can take a week, occasionally longer. Once the immigration medical examination is completed and all immunizations and/or lab tests have been performed and completed, we will provide you with a certified form I-693 in a sealed envelope to present to the Department of Homeland Security and U.S. Citizen and Immigration Services. DO NOT OPEN THE SEALED ENVELOPE. Turn in the envelope with your immigration application. A copy of your blood tests results and exam will be provided for you to keep after results have been discussed with the doctor. The medical examination will only be valid up to a year after the immigration exam date.
Typically, you will be notified of the results of your lab work (and chest x-ray, if done) within 7-10 days. If you have a health condition requires that medical follow-up, clearance will not be granted until follow-up is completed. You will be responsible for the cost of any additional medical examinations or treatments required to fulfill the federal health requirements established by USCIS.
It is your responsibility to bring a translator and have all your necessary immunization and medical records translated into English to expedite your exam process.

From acute injuries on the ball field to weekend warrior mishaps- sprains and strains, bumps, bruises, stitches, staples and IV fluids are some more of the ways we excel at ExperCARE. We even have an in-house x-ray, labs, EKG’s, and much MORE.
Routine Care
Don’t have a doctor? Can’t get a convenient appointment?
Don’t want to wait to find out? Need non-controlled medications refilled? No problem! We are great at assessing just about any medical complaint, ordering necessary testing and providing a treatment plan customized just for you! We can even send you to the appropriate specialist for follow-up if indicated.
Labs, X-rays, Diagnostics
We offer in-house x-ray services and a full range of lab testing for all of your diagnostic needs. We have a board-certified radiologist on call for our providers all hours that we are open to provide the highest level of care and service for our patients. Need a vaccination or flu shot? We’ve got you covered. We even offer the option to fill most commonly prescribed medications on-site in minutes, for an affordable cash rate.
Occupational Medicine
We offer a full range of occupational health services delivered by experienced staff, knowledgeable in work-related injury care. Our team is trained to work with employers and employees to minimize lost time and recordability and optimize employee wellness. We specialize in delivering extraordinary care to employees while communicating with the employer to ensure a seamless process. We have FMCSA Certified Medical Examiners available on a walk-in basis daily and our centers are members of DATIA with NIOSH certified experts on staff.
